Service Description:

Integrated, countywide, 24-hour mental health service plan system offered to adults with serious mental health concerns
Client-centered voluntary service * provides crisis response, case management and supportive housing with a team of professional staff trained in a variety of disciplines including social work, mental health nursing and occupational therapy
Case Management - Offers individualized community-based support for people with a serious mental illness * based on individual need and is client directed * goals of case management are: to assist people to live as independently as possible, to assist people in making their own decisions, and to assist people in accessing a range of resources that will enable them to realize personal goals and acquire skills needed to be successful * offers life skills training on an individual basis or within a small group format * information, assistance and encouragement are provided to promote the development of skills necessary for independent living
Case management service is provided in the individual’s home or a place of their choice within the community
