Service Description: Health centre provides a wide range of health services to the community with a focus on disease prevention and health promotion * services provided by registered nurses and other allied health care providers

Community Health Nurse - Provides public health programs, including health education, vaccination and disease prevention services

Community Health Representative - Coordinates health services including blood pressure and blood sugar screening, foot care, workshops on healthy lifestyles, diabetes, smoking cessation and healthy choices for youth

Visiting Health Professionals - Visits made by dentist, doctor, psychologist, elders, healers and foot care specialist on a regular basis

Diabetes Education Program - See Diabetes Grey Bruce
 
Community Support Services - Meal services and activities for seniors
 
Environmental Program - Monitors and coordinates the West Nile Virus program and water testing * manages other environmental health related issues
 
Home and Community Care - Home care program * needs assessments conducted by community nurses * personal support workers provide personal care and homemakers provide housekeeping services
 
National Native Alcohol and Drug Abuse Program - Addictions assessment, counselling and referral services
 
Non-Insured Health Benefits/Medical Transportation - Coordinates appeals and exemptions to health benefits * provides transportation to medical appointments
 
Youth Activities and Addictions Prevention Program - Drug and alcohol prevention program for children and youth
