Service Description:
|
Adult day program for frail elderly, adults with physical disabilities and individuals with Alzheimer's Disease and related conditions
Provides social, recreational and therapeutic activities in a supportive group setting
Services include blood pressure check, weight monitoring and bathing in a secure facility * medical monitoring provided by RN or RPN at each site * coordinator provides an in-home assessment prior to placement and provides ongoing support to the family caregiver * individual social/recreation plans developed for each client * hair care and foot care available for a fee
Additional Service Details
- Transportation - volunteer drivers or staff pick up clients in the morning and bring them home in the afternoon * arrangements made by staff
- Nutrition - light refreshments served in the morning (10am), lunch at noon * special dietary needs accommodated (diabetic, minced personal preferences)
- Mobility Assistance - staff are trained in transfers, escorting and falls prevention * spaces are accessible * clients with canes, walker, wheelchairs and other mobility issues accommodated
- Incontinence - provides support to clients with bladder incontinence * clients encouraged to wear briefs or pads to support bladder control and invited to bring a spare change of clothing to be stored at the program
- Medication Management and Medical Care - administers and assists client with medications required * medication must be in original dispensing container indicating name, medication, dosage and special instructions * up-to-date list of medication taken at home and at Day Program is required and is responsibility of client/caregiver
- Overnight Respite - available once a month in Wiarton site and once a month in Hanover site * room for 2 clients per night with 2 PSW staff on duty * program runs from 4:30pm - 8:30am * must be scheduled ahead of time with staff
- Expanded Hours - 9:30am-3:30pm * expanded hours available by special request
- Wandering/Exit Seeking - program has a secure locked entrance
- Specialization - an integrated program to manage clients with various physical and cognitive challenges
