Provides comprehensive community health, support and in-home services by nurses, personal support workers and volunteers

Foot Care
- Primary and advanced foot care services provided by registered nurses and registered practical nurses

Home At Last
- Identifies older adults in local hospitals who have limited supports upon discharge or potential concerns regarding how well they may settle when they return home
|

Nursing Services
- Registered nurses with community education and experience provide nursing care to clients and support to caregivers in the community - at home, at school, at work

Personal Support
- Private personal care and essential homemaking to clients and support to caregivers
