Service Description: Programs to support independent living for seniors and adults with disabilities
 
Adult Day Program - Dorchester - Therapeutic activities in a group setting for adults, those with physical disabilities or those with cognitive impairment such as Alzheimer's, dementia or a brain injury
 
Adult Day Program - Strathroy - Therapeutic activities in a group setting for adults, those with physical disabilities or those with cognitive impairment such as Alzheimer's, dementia or a brain injury
 
Alzheimer/Dementia Services - Offers a variety of services to individuals with Alzheimer’s disease and related dementias and their Caregivers individual support 
 
Blood Pressure Clinic - Trained volunteers help clients measure seniors blood pressure and provide educational materials and information on how to be more heart-healthy
 
Caregiver Support and Counselling - One-on-one and group support, as well as counselling for caregivers and families, providing them a chance to share feelings, problems, ideas, and information with others
 
Caregiver Training, Information, and Education - Peer support groups, education and training assist family members, service providers and caregivers
 
Home Maintenance and Referral - Connects seniors or adults living with a disability with companies and/or individuals to provide home maintenance, housekeeping, meal preparation, yard work, etc
 
Hospice and Bereavement Services - Companionship, respite care and emotional support to people and their family/caregivers dealing with a life threatening illness * support in all settings - community, hospital or Long Term Care
 
Hospice Respite Visiting - Trained volunteers provide support and respite to individuals and their families who are facing life-threatening or life-altering conditions, as well as those who have lost a loved one
 
Lunch Out, Congregate Dining, Lunch Bunch - Provides a breakfast or lunch and entertainment for seniors or adults with disabilities * various locations across Middlesex County
 
Meals on Wheels - Volunteers deliver hot or frozen meals to clients helping to meet their nutritional requirements * hot meals delivered at lunch * supper available at some locations
 
SMART Exercise Clinics - Certified volunteer instructors lead weekly, one-hour, high or low mobility exercise classes geared towards seniors and adults with a disability
 
Telephone Reassurance/Security Checks - Regular telephone call or other contact by a volunteer for residents living in apartment buildings to ensure the client is not in crisis
 
Transportation Services - Volunteer transport and accompaniment for seniors and adults with disabilities to get to appointments, shopping, errands, and social outings
 
Vial of Life - Trained volunteers assist clients to complete medical information in a provided template, so it is accessible for emergency personnel, in the event of an emergency
 
Volunteer Visiting - Volunteer visitors are matched one to one with seniors and adults with disabilities to provide companionship and support
