Service Description:
|
Offers therapeutic, recreational programming for people with Alzheimer's disease and related dementias in a secure environment * 55-60 spaces daily
Transportation Services: transportation is door to door for those living in London * extended hours available for supper and activities * busing available for extended hours
Transportation - door-to-door transportation
is available through the Boys’ and Girls’ Club within the London area
and from Lambeth * buses transport clients to the programs at
9:30 am and 2:30pm and return them home at 3pm, 6:30pm
and 8pm
Nutrition - Nutritious meals are provided for
clients, including breakfast, lunch, dinner and refreshments depending
on the time period the client is present in the program * meals are
prepared by our food services department and meet standards mandated for
long-term care * special dietary needs can be accommodated, including
requests for diabetic, puréed or vegetarian meals.
Mobility
Assistance - fully accessible * staff
are trained in falls prevention and transfers * clients are welcome to
bring their mobility aids, such as canes, walkers and wheelchairs * ceiling lift is available in one of our washrooms.
Incontinence -
Personal Support Workers are available to assist clients with
incontinence issues * Incontinence products are to be provided by the
client/caregiver * we recommend supplying a spare set of clothing to the
program.
Medication Management and Medical Care - Registered
nursing staff are available from 8:30am-6pm to administer
medication * we ask that
prescription medication be supplied in clearly marked pharmacy
containers or preferably in blister packs directly to program
staff * delivery of the drugs by the clients themselves is discouraged * we strongly encourage families to keep the registered nursing staff
informed of all changes in medication.
Overnight Respite – five
overnight respite beds are available to clients on Friday, Saturday and
Sunday nights * they are available to clients in the day program but
those clients not using the regular day programs can also be
accommodated with certain conditions.
Expanded Hours - program offers extended hours for 15 clients, seven days per week * clients who arrive at 9:30am can pre-arrange their stay until 6:30pm for a fully staffed and activity-based extended program with
dinner served at 5pm.
Wandering/Exit Seeking - program
area is secured by magnetic locks that are released by a number pad * these locks automatically release in case of an activated fire alarm.
Specialization
- offers specialized services for clients with Alzheimer's
disease and related dementias customized to a person’s
skills, background, interests and abilities. * staffing levels and physical
environment allow programming to take place in smaller groups and at the
most appropriate level to the individual client’s abilities.
Unable to accommodate clients who are medically unstable or require a high level of nursing care * aggression toward
other clients or staff that cannot be mitigated by best practice
approaches may lead to temporary or permanent discharge from the
program * Some behaviors that cause distress to other program participants that cannot be mitigated by
staff interventions may also result in discharge * clients who
have minimal or no ability to bear any weight will be assessed on an
individual basis to determine if their needs can be met by the program.
