Service Description: Support adults with a serious mental illness and/or a developmental disability by providing quality services that foster personal growth and participation as full citizens in their communities
 
Apartment Living - Provides supports to individuals with a serious mental health illness or a dual diagnosis living in 8 unit apartment building located in a rural setting
 
Connections - Provide training and vocational opportunities to adults with mental health and/or developmental disabilities
 
Developmental Services/Community Living - Residential supports are provided to adults who have a developmental/intellectual disability  * leisure, recreation, vocational services offered * Supported Independent Living supports (SIL) are also offered
 
Group Living - 12-bedroom co-ed group home providing housing for 12 individuals with a dual diagnosis and or a serious mental illness
 
Support within Housing/Community Supports - Individualized support and services are provided to persons with serious mental health illness to help them to live independently
Fees: Individuals are responsible for living expenses
Application: Self referral or service provider referral
Eligibility - Population(s) Served: Adults 18 years and older with an intellectual disability and/or serious mental health issues
