Service Description: Ministry of Health and Long-Term Care funded Diabetes Education Program

Diabetes care team offers monthly groups that give participants reliable, up-to-date information about learning to care for themselves

Focus is on prevention and self-management * support is available in groups but also one-on-one with a diabetes educator, community worker or dietitian  *multicultural diabetes program available * take a self-test www.areyouatarget.ca

Application: Self referral - telephone or drop-in * appointment required for some programs or telephone for referral * call to register for group meetings
Eligibility - Population(s) Served: Anyone from London with diabetes or who is at risk of diabetes
Languages: English
