Service Description: Provides health and mental health services combining traditional healing and western medical practices * offers primary health care, mental health and crisis intervention, diabetes education and awareness, nutrition and physical activity, etc. * information and referral to services available on and off reserves

Primary Health Care - Physicians and nurse practitioners working to promote wellness, prevent illness, provide care for common concerns and manage ongoing chronic conditions

Nutrition and Healthy Lifestyle Program - Provides nutrition education and promotes active living

Supporting Aboriginal Seniors at Home (SASH) - Provides improved access to primary care and chronic disease management to Aboriginal seniors 55 years and over who are living in the London area both on and off Reserve * goal is to provide culturally safe care for Aboriginal seniors and to link clients with the health and community supports required for to live safely in their homes

Diabetes Education - Ministry of Health and Long-Term Care funded Diabetes Education Program * provides education and support for people living with Type 2 Diabetes or pre-diabetes
 
Maternal Child Health Nutrition Program - Provides nutrition and FASD education programs for the development of healthy families and children * Group education only on topics as follows: nutrition for children and youth, pregnancy, breastfeeding, general healthy eating, healthy lifestyles and wellness, shopping and cooking, and education of Fetal Alcohol Sprectrum Disorder
 
Mental Health Counseling Program - Provides holistic and professional counseling services to FNMI individuals, including: children, youth, adults, and elderly
 
Traditional Healing Program - Utilizes local Indigenous healing methods to provide a traditional, (w)holistic approach to individual, family and community health and wellness

Application: Self referral - telephone or drop in * bring your health card, certificate of Indian status (if applicable) and list of medications on first visit
