Service Description:
|
Offers a range of in-home health care services to help seniors and individuals with disabilities remain in their home
Personal Care Services - morning and bedtime care, exercise support, help with personal care, medication reminders, personal care and grooming, catheter and perineal care
Care Management Services - Manages health care needs and caters to physical, mental and emotional needs * offers an in-home assessment and consultation, supervisory safety visits * develops and monitors a care plan with 24-hour on-call support
Home Cleaning and Maintenance Services - Services may include dusting, cleaning, vacuuming, changing bed linens, laundry, pet care, plant care, seasonal outdoor jobs Companion Care Services - Social interaction with a friendly companion * clients are matched with a companion based on personal and social preferences * companion may assist with errands or transportation
Foot Care - Includes nursing assessment to identify risk factors and foot problems, foot massage, nail cutting, corn and callous care, ingrown toenail care * assessment for specialized treatments and referrals to appropriate professionals in the community
Home Medical Equipment and Supply Coordination - Offers advice and coordination of purchase for medical equipment and supplies * will arrange for an in-home specialist assessment and ensure products and services meet home environment needs
SMART Personal Emergency Response System - provides sales and rental of a personal alarm system to call for help in an emergency
Nursing Care Services - Provides supervision and regular nursing procedures to improve quality of life * includes medication administration, vital signs monitoring, wound care and dressings, catheter care, palliative services * communicates with health care professionals as required
