Offers a range of in home health care services to help seniors remain in their home * approved care provider for Veterans Affairs
Personal Care Services
- Offering morning and bedtime care, exercise support, help with
personal care, medication reminders, personal care and grooming,
catheter and perineal care
Care Management Services -
Manage all your health care needs * services cater to physical, mental
and emotional needs * offers an in-home assessment and consultation,
supervisory safety visits * develop and monitor a care plan, 24-hour
on-call support
Home Support Services - Services may include dusting, cleaning, vacuuming, change bed linens, laundry, pet care, plant care, seasonal outdoor jobs
Companion Care Services - Social interaction with a friendly companion * clients are matched with a companion based on personal and social preferences * in the home or hospital
Nursing Care Services - Provides supervision and regular nursing procedures to improve
quality life * includes medication administration, vital signs
monitoring, wound care and dressings, catheter care, palliative services
* communication with health care professionals as required
Foot Care -
Includes nursing assessment to identify risk factors and foot problems,
foot massage, nail cutting, corn and callous care, ingrown toenail care
* assessment for specialized treatments and referrals to appropriate
resources in your community
Home Medical Equipment and Supplies - Offer advice and coordination for medical equipment and supplies
including incontinence products, walkers, wheelchairs, scooters, canes,
grab bars, commodes ,bathroom safety products mechanical lifts, beds,
protective equipment, medication delivery * will arrange for an in home
specialist assessment and ensure products and services meet home
environment needs
