Service Description:
|
10 bed unit to help patients regain their independence after an acute illness hospital stay * patients stay in hospital for two to eight-weeks to improve function so they can return home to live independently
Patients care for themselves (dressing, bathing, taking medication) at the hospital, with assistance if required, and participate in daily activities designed to regain strength and independence * the support team includes an Nurse Practitioner, Occupational Therapist, Physiotherapist, Nurses, Personal Support Workers (PSW) and an Occupational Therapy/Physiotherapy Assistant * patients can also access dietitian, social work, wound care and foot care services

A Grey Bruce resident, 18 years or older who is an inpatient in a Grey-Bruce hospital
recovering from an acute illness; stable with a manageable health condition
