Service Description:
|
Provides treatment planning and community-based outpatient treatment for individuals with a substance abuse problem
Offers treatment models based on cognitive-behavioural, stages of change, harm reduction, multidisciplinary, and solution-focused * offers case management, outpatient counselling and aftercare to facilitate recovery * services include:
- ongoing assessments and ongoing adjustment of the treatment plan
- linking to and coordinating required services
- monitoring and supporting
- developing and implementing the discharge plan
- and advocacy
Treatment sessions assist the individual to develop skills to manage substance abuse and related problems, and/or maintain and enhance treatment goals * activities such as relapse prevention, guided self-change, follow-up and aftercare are included as well as programming to support/educate family
Support Groups:
- Men’s Group (open format)
- Women’s Group (open format)
- Relapse Prevention Group (close format)

Adults and youth who are affected by substance use, abuse and dependence * clients with minor mental illness are accepted
