Service Description: 20-bed acute and complex continuing care hospital with emergency department * Care Coordinators from the South West Local Health Integration Network - Home and Community Care available on site

Complex Continuing Care - 10 inpatient beds for patients who require ongoing monitoring of care that can only be received in hospital or for patients waiting for discharge to another setting * individuals are assessed with respect to the level and type of rehabilitation services they require * care is planned and delivered based on goals identified in collaboration with the interdisciplinary team and their families
 
Huron Perth Diabetes Education Program - Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff
 
Outpatient Clinics - Outpatient specialist clinics providing diagnoses and treatments for various medical conditions

Fees: Most fees are covered by OHIP
Application: Walk in for emergency department * medical referral and appointments required for most other services
