Service Description: Supports for people living with prediabetes or diabetes, or at-risk * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * team includes certified diabetes educators, registered dietitians, foot care specialists and registered nurses

Self-management groups offered on monthly basis, including linguistic and culturally-specific groups:
  • Latin American
  • Cambodian
  • Korean
  • Arabic
  • Black Canadian
  • Southeast Asian
  • Persons on a low income

Fees: Free
Application: Call * no referral required
Eligibility - Population(s) Served: Adults 18 years and older with pre-diabetes, risk factors for diabetes, or type 2 diabetes

Language Note: Interpretation available upon request * groups offered in a variety of languages
