Service Description: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible

Companionship, respite care, and emotional support to people and their family/caregivers dealing with a life-threatening illness * bereavement support for anyone who has experienced the death of someone close * services can be provided at home, or in hospitals and long-term care homes

Resource Library - End-of-life care collection available for loan * includes literature, audio, and video

Bereavement Services - Support groups for all ages held in various communities throughout the year * education about the grief process for both children and adults

Hospice services begin at the time of diagnosis
 
Bears and Kits for Kids - Program for children dealing with a life threatening illness, either their own, a family member or a friend * support also provided for children that have experienced a loss or death * items are designed to facilitate discussions of feelings and to encourage and develop a healthy understanding of dying and grief
 
Grief and Bereavement Outreach - Provides education  and one-on-one support to both the caregivers, families and the bereaved about the grief process
 
Visiting Volunteer Program - Psychological and emotional support for anticipatory, acute and chronic grief for people facing the challenge of a life-limiting illness
