Service Description: Ministry of Health and Long-Term Care funded Diabetes Education Program * provides education and support for people living with Type 2 Diabetes or pre-diabetes

Team includes a Diabetes Nurse Educator, a Registered Dietitian and an RPN specializing in foot care * work with people to set personal goals that can help them live a healthier life * available at both the Chippewa and London sites
Application: Self referral - telephone or drop in * appointment required
Eligibility - Population(s) Served: First Nations, Inuit and Métis community, on or off the reserve
