Service Description: Offers primary care services and health programs to members of the community * staff includes doctors, nurse practitioners, registered nurses, social workers, physiotherapist, registered dietitian, chiropodist, health promoter and community developer * currently accepting new patients

Community based programs and services for patients and the community:
  • Smoking cessation program providing individual support for people wanting to reduce or quit using tobacco
  • Offers Living a Healthy Life with Chronic Conditions - self-management workshops
  • Support provided to help fill out government forms, call for appointment
  • Rideshare Bulletin Board
  • free phone and computer/internet access
 
Community Food Programs - Provides a variety of community food programs for community members
 
Fitness and Recreation Programs - Provides a variety of fitness and recreation programs for community members
 
Foot Care Services - Chiropody clinic specializing in assessment and treatment of common foot problems * provides foot care
 
Individual and Family Counselling - Mental health counsellors provide free, confidential, individual, couple and family counselling
 
Nutrition Services - Registered Dietitian provides information about food and nutrition
 
Physiotherapy Services - Physiotherapist works with clients to identify areas of concern and to implement strategies to help alleviate some of the symptoms associated with injury, illness or disability
 
Student Health Clinic - Health clinic for students attending Grey Highlands Secondary School

Application: Health Care Services - Complete the Request for a Family Doctor or Nurse Practitioner form * some specialized services can be accessed by non-patients (dietitian, physiotherapy, chiropody) with referral from their health care provider
