Service Description: Health centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups

Community-led health teams develop programs to improve the social supports and conditions that affect long-term health

Primary Health Care Services -
Health care team includes doctors, nurse practitioners, registered nurses, dietitian, physiotherapist, social workers, chiropodist (foot care specialist), health promoter, and community developer (doctor and nurse practitioner services available to registered patients * some services open to everyone)

Chronic Disease Self-Management - Sessions to learn ongoing self-care * includes information about coping, health guidelines, medication usage, working with health professionals, and informed treatment decisions (open to everyone)

Falls Prevention Program -
Group falls prevention classes for older adults and people with mobility issues * aims to prevent injury and falls (open to everyone)

Smoking Cessation - One-on-one support to help quit smoking (open to everyone)

Physiotherapy Services - Physiotherapist works with clients to identify areas of concern and to implement strategies to help alleviate some of the symptoms associated with injury, illness or disability
