Service Description: Community health care agency * Personal Support Manager available to complete free in-home assessment to establish care plan that meets patients needs to remain independent in own home.

In-Home Personal Support Workers provide:
  • Bathing/assistance withpersonal care
  • Dressing assistance
  • Mobility assistance
  • Meal preparation assistance

In-Home Nursing provides:
  • In-home assessment (such as falls hazards)
  • Wound care
  • IV therapy
  • Catheter
  • Drain care
  • Feeding tubes
  • Disease self-management training

Nursing services are available in designated clinic locations
