Service Description: 5-bed unit to help patients regain independence after an acute illness hospital stay * patients stay in hospital for two to eight-weeks to improve function so they can return home to live independently

Patients care for themselves (dressing, bathing, taking medication) at the hospital, with assistance if required, and participate in daily activities designed to regain strength and independence * support team includes an occupational therapist, physiotherapist, nurses, personal support workers and an occupational therapy/physiotherapy assistant * patients can also access dietitian, social work, wound care and foot care services

Application: Medical referral – apply through Community Care Access Centre * discharge planners in hospital will assess the patient's eligibility for the program
Eligibility - Population(s) Served: Grey Bruce residents 18 years or older who are inpatients in a Grey-Bruce hospital and recovering from an acute illness * must be stable with a manageable health condition
