Visiting Hours: 2pm-4pm and 7pm-8pm
Service Description: 5-bed unit to help patients regain independence after an acute illness hospital stay * patients stay in hospital for two to three weeks to improve function so they can return home or to lodge/retirement home to live independently (with family and home care support as required)

Patients progress to care for themselves (dressing, bathing, mobility) at the hospital, with assistance as required, and participate in daily activities designed to regain strength and independence

Support team includes an occupational therapist, physiotherapist, nurses, personal support workers, dietitian, and occupational therapy/physiotherapy assistant

Application: Medical referral – apply through Community Care Access Centre * discharge planners in hospital will assess the patient's eligibility for the program
Eligibility - Population(s) Served: Grey Bruce residents 18 years or older who are inpatients in a Grey-Bruce hospital and recovering from an acute illness * must be stable with a manageable health condition
