The West Elgin Community Health Centre is part of a strong network of community health centres across the province. Together we commit to providing health and social services that are accountable, accessible, efficient and governed by the community.

Our team provides primary health care, illness prevention and health promotion services, as well as a range of community programs to the residents of Dutton Dunwich and West Elgin municipalities. Our diabetes services are open to anyone and are offered at multiple locations in Elgin County.

Position:  Nurse Practitioner - Primary Care

Region:  Elgin

Description:  The Nurse Practitioner provides primary care functions and participates in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community.

Qualifications:

  • Current Registered Nurse, Extended Class (RN-EC) in good standing with the College of Nurses of Ontario
  • Experience working in rural communities preferred
  • Strong assessment and organizational skills
  • Demonstrated ability to work collaboratively in a team setting

Responsibilities:

  • Perform assessment, diagnosis, treatment, health education/counseling and other clinical activities according to the College of Nurses of Ontario standards of practice for Registered Nurses in the Extended Class (RN-EC)and in accordance with Centre Policies and Procedures.
  • Consult internally with family physicians when client needs fall outside of scope of practice.
  • Be aware of and to work towards the achievement of the performance indicators and outcomes identified in the SW-LHIN Accountability Agreement and the WECHC Strategic Plan related to this position.


Salary:  $98,001

Terms of Employment:  Contract
