Salvation Army - London Village

The Salvation Army London Village is a Christian Community dedicated to providing support to families through child care programs for children from birth to 12 years,
respite programs for children and adults with intellectual disabilities, and Adult Day Programs for adults with Alzheimer Disease or related dementias.


Position:  Adult Day Program Community Nurse

Region:  London and Middlesex

Description:  We are seeking a Community Nurse who will work in our Adult Day Program.

Responsibilities:

  • Direct the program staff in the absence of the Adult Day Program Director as per established guidelines when required
  • Provide care in accordance with administrative policies, the College of Nurses of Ontario “Standards of Nursing Practice”, the standards of Gerontological Nursing
  • Maintain program capacity, efficiently managing client admissions and discharges, as required, by:
    • Receive referrals from the Community Care Access Centre and ensure a timely response to all inquiries
    • Determine appropriateness of referrals/redirect to alternative services when necessary.
    • Review all preliminary client assessment information e.g. (RAI) and coordinate intake appointment with client/caregivers.
    • Conduct onsite appointments to assess client suitability and complete required intake documentation
  • Develop individual client care plans in conjunction with the ADP team
  • Coordinate transportation services with transit service provider
  • Receive and document all medications upon arrival and ensure safe and secure storage is maintained at all times. Confirm dosages and instructions for all medications and treatments prior to administration. Administer and record the provision of all medications and treatments as performed
  • Work cooperatively with Administrative Assistant to ensure accuracy of client data base and billing information is maintained
  • Perform end of day facility closure routine
  • Receive the all clear notification from transit service provider and respond to any end of day transit related issues
  • Assist with the arrival and departure of participants and follow up with any absences or health related concerns
  • Assess and monitor the condition of participants through recognizing and interpreting signs/symptoms and report concerns to family care providers
  • Monitor nutritional status of participants, liaise with the ADP Program Director, and care providers to resolve related concerns and to assure appropriate meals are ordered. Inform staff, students, and volunteers of any dietary restrictions or special meal related instructions. Submit weekly meal order to contracted service provider and submit related information to Director as requested
  • Provide direction to the PSW staff and assist with the distribution of refreshments /meals, feeding of participants, set up and clean up as required. Familiarize self with all new client information and share with staff prior to start date.
  • Liaise with practicum placement facilitators, coordinate, supervise, and evaluate PSW students while on placement
  • Perform foot care as requested by participants/families. Assist PSW with scheduling and provision of baths as required. Coordinate and assist with the provision of personal hygiene routines. Maintain accurate daily attendance, foot, hand/nail care and bathing records, summarize and submit to ADP Program Director monthly
  • Provide support and assistance during all staff absences
  • Perform lifts and transfers, transport and assist clients with mobility deficits
  • Orientate and provide ongoing training to Personal Support Workers regarding their role and the safe use of all equipment and supplies
  • Effectively manage difficult client behaviors, outbursts, and altercations
  • Liaise with emergency personal and care providers during participant related medical emergencies
  • Provide appropriate client intervention and leadership to team members during crisis and emergency situations
  • Complete and submit monthly safety inspections. Facilitate and document fire drills for Cottage #4
  • Facilitate the team Client Reassessment process
  • Monitor the environment for risk and safety issues, implement appropriate actions, and report concerns to ADP Program Director
  • Assist in the promotion of a healthy physical and psychosocial environment for the clients by leading social/recreational activities upon request
  • Maintain accurate and complete participant records of nursing observations and care/treatments/medications provided and any other pertinent information
  • Maintain the participant emergency file
  • Perform other position related duties as required

Qualifications:

  • Completed a two (2) year Community College diploma program in nursing, degree preferred
  • Current, valid registration as a Registered Practical Nurse with the College of Nurses of Ontario
  • Bilingual in French preferred
  • Minimum of two (2) years of prior related nursing experience with the cognitively impaired, frail elderly population
  • Demonstrated case management experience working with the frail elderly/cognitively impaired client populations
  • Exceptional written and verbal communication and interpersonal skills for relationship building, consultations, and problem solving with external organizations, professionals, applicant’s families and agency staff
  • Strong computer skills, including proficiency in Word, Excel, PowerPoint and ACCESS
  • General knowledge of the Public Guardian and Trustee Act, Powers of Attorney and Advanced Care Directives
  • Knowledge of and/or use of the inter RAI and inter RAI CHA assessment tools and processes
  • Current certification in First Aid & CPR
  • Valid Foot Care Certificate with recent experience
  • Safe Food Handlers Certificate
  • Crisis Prevention Institute (CPI) Certification or Gentle Persuasive Approach Training
  • U-First Training is preferred
  • P.I.E.C.E.S training is preferred
  • Comprehensive knowledge of various forms of dementia
  • Ability to recognize and respond to complex situations
  • Capable of establishing and maintaining positive interpersonal relationships and able to work as an effective team member
  • Demonstrated ability to follow directives, be self- directed, disciplined and to direct others
  • Ability to apply effective body mechanics and meet the physical demands of the position
  • Provide an original copy of a Background Check with vulnerable sector screening that is satisfactory to The Salvation Army, in its sole discretion, is required. The screening is secured either through the National Canadian Policy Information Centre (CPIC) or through a local police detachment
  • Support for and an understanding of the mission and purpose of The Salvation Army in Canada


Salary:  $27 per hour

Terms of Employment:  Casual
