Heroes in the Home 2014

Click here to nominate a special caregiver!

Help the South West Community Care Access Centre and SouthWesthealthline.ca celebrate a special caregiver in your life at a Heroes in the Home event! Each year, CCAC clients and all others involved in home and community care have the chance to nominate a special caregiver – whether a family member, friend, volunteer, or a health care professional – to receive recognition for going above and beyond the call of duty.

Every Hero will be recognized with a Certificate of Recognition and be honoured in person during a Heroes in the Home ceremony in their region.

The events will take place from 3:30 - 5:00 p.m. on:

Wednesday, October 8, 2014 in Owen Sound to recognize caregivers from Bruce and Grey counties
Nominations due September 5, 2014

Wednesday, October 22, 2014 in London to recognize caregivers from Elgin and Middlesex counties and the city of London
Nominations due September 19, 2014

Wednesday, November 12, 2014 in Stratford to recognize caregivers from Huron, Perth, Oxford and Norfolk counties
Nominations due October 10, 2014

If you would like to honour a special caregiver, please complete this online nomination form. If you would prefer a hard copy, you can click here to download the form.
