In 2012, more than 600 extraordinary people were nominated across the South West as Heroes in the Home. The program, now in its tenth year, recognizes professional care providers and friends and family members who support people to live at home.

The Heroes in the Home program has proven so successful it has been adopted by several other CCACs across Ontario. In September it was recognized as a Leading Practice by Accreditation Canada.

Each year, the CCAC calls for nominations for Heroes in the Home from anyone in the community. Every nominee is honoured at special events held in London, Stratford and Owen Sound. Check back here in the Fall of 2013 for next year's nomination form.

Every day, people in our communities provide wonderful care to those facing the challenges of age, illness and disability. The South West Community Care Access Centre celebrates these “Heroes in the Home” through the Caregiver Recognition Awards.

In 2011, almost 500 caregivers were honoured across the South West region at three Heroes in the Home ceremonies.
