Client-Driven Care


Welcome to our library of online Client-Driven Care resources!

Client-Driven Care (CDC) is an approach to working with clients, their families and other care providers. It involves working as a team, respecting each other’s knowledge, building on each other’s strengths, and working together to reach better health outcomes. CDC makes for more confident and capable clients and more satisfied healthcare providers.

CDC was developed and piloted in the South West and is now part of the ethos of the South West Community Care Access Centre. The practice of CDC is relevant to many different healthcare settings.

This library includes materials developed as part of a CIHR-funded project on knowledge translation, and other training and information materials.

These resources visualize and describe CDC, and provide opportunities to try out new approaches to care:
