Various programs and services support the important decisions you need to make about the future of your health, and health care.

Advance directives and living wills
record your treatment wishes in case of an injury or health condition where you become incapable of making the decision yourself.

A Power of Attorney is a legal document naming a specific person to act on your behalf, sometimes called a ‘substitute decision maker‘. Your treatment wishes may also be recorded in this document. See the Ontario Ministry of the Attorney General record, below, for more information.
