Community Health Centres - South West

Community Health Centres provide primary care, health promotion, education and illness prevention services using a community development approach. Health care professionals including physicians, nurse practitioners, nurses, counsellors, community workers and dietitians staff community Health Centres.

Please be advised that community health centres are not walk-in clinics. Services are available for currently rostered patients.
