All forms are located under the “SUBMIT YOUR CONTENT” menu on the right-hand side of the page. If you need help filling out the forms, please email email@example.com or contact us.
Once a form is submitted, we will review and post your information on SouthWesthealthline.ca. This process typically takes one to five
business days. Once your information is posted, e-mail firstname.lastname@example.org
or contact us
to make any changes.
If we are not able to post your information because it does not fit our
inclusion criteria, we will contact you.

To make the service profile updating process easier, we have
created a tool that allows organizations with multiple service profiles to view
all of their service profiles in one place and to update information at one time. Please
use the Health Care Provider login below to access this tool.

fields for health care providers and planners. These may include more detailed information
about services and the application process. This information can be viewed by using the Health
