Service Description:

Intensive case management services to people with a serious mental illness to assist with skills to live independently
Case
managers provide advocacy and one-to-one support to people learning to
live with and manage their mental well-being * Case managers can assist
with improving interpersonal relationships, budgeting, finding housing,
and accessing other community supports and services

Eligibility / Target Population:
|
Adults and youth 12 years of age and older who are affected by substance use and dependence * clients with minor mental illness are accepted
|
