Service Description:

Affordable transitional housing up to 4 years that provides opportunities for people with a longstanding serious mental illness and/or substance use involvement to stabilize and establish connections with the community- assessment and planning
- support with achieving recovery directed goals
- direct service provision/intervention
- monitoring, evaluation, and follow-up
- information, liaison, advocacy, consultation, and collaboration
- planning for crises and helping individuals to develop coping mechanisms

Free * rent geared-to-income for residences

Application and referral required for geared-to-income housing * self or other service provider referral for outreach support

Eligibility / Target Population:
|
Adults 18 years of age and older who:
- want to make a commitment towards mental health and/or substance use recovery
- are willing to regularly engage with the worker (this is a requirement)
