Service Description: Affordable transitional housing up to 4 years that provides opportunities for people with a longstanding serious mental illness and/or substance use involvement to stabilize and establish connections with the community
  • assessment and planning 
  • support with achieving recovery directed goals
  • direct service provision/intervention
  • monitoring, evaluation, and follow-up
  • information, liaison, advocacy, consultation, and collaboration
  • planning for crises and helping individuals to develop coping mechanisms

Fees: Free * rent geared-to-income for residences
Application: Application and referral required for geared-to-income housing * self or other service provider referral for outreach support
Eligibility / Target Population: Adults 18 years of age and older who:
  • want to make a commitment towards mental health and/or substance use recovery
  • are willing to regularly engage with the worker (this is a requirement)

Language Notes: Attempts will be made to provide service in other languages as requested
