Service Description:

Community Health Centre that provides primary health care and community support services to individuals and families in the community, including underserved and vulnerable populations.
Services focus on health promotion, disease prevention, early intervention, and improving access to care * works to reduce barriers to healthcare and support overall health and well-being through accessible and coordinated services.

- Nutrition services for all life stages * personal support in group and/or one-to-one sessions with a certified dietitian * programs include:

- Outreach staff assist with system navigation, community resources, transportation, advocacy and filling out forms

Youth Outreach and Support Coordinator
