Service Description: Project designed to improve outcomes for people who have had a stroke or TIA (transient ischemic attack)

To be completed by 2017 in two phases:

Phase I -  Realigning delivery of inpatient acute and rehabilitative inpatient care from 28 hospital sites to 7 Designated Stroke Centres

Phase II – Creating recommendations to enhance care for TIA and stroke survivors after they leave the hospital * includes secondary stroke prevention, outpatient and community rehabilitation, and stroke recovery services
