Service Description:

In partnership with St Mary’s General Hospital, Hanover and District Hospital, and Diabetes Grey Bruce, patients who have had a cardiac event or who have certain cardiac risk factors will be assessed and treated by members of the multidisciplinary team according to patients’ needs
