Service Description:

Program supports children under the age of 6 who do not have a primary care physician or nurse practitioner
Services include:- Well Baby/Child Assessments
- Immunizations Developmental
- Screening Nutrition
- Support Referrals
- Assistance with navigating the healthcare system and community services
- Assessment and treatment of non-emergent, episodic concerns such as ear infections, cough, rashes

No referral required * applications may be made directly through provided number and extension or WECHC website

Eligibility / Target Population:

Children under 6 years old unattached or "questionably" attached, i.e. provider not accessible due to distance
