Service Description:

Part of the Senior Centre of Care * 5-bed inpatient program * short-stay (2 to 3 weeks) restorative rehabilitation program to help patients return to their home after an injury or illness * care includes exercise and recreational activities to increase strength and independence

Eligibility / Target Population:

Medically stable rehabilitation patients who are mentally and physically able to participate * patient's anticipated discharge destination cannot be a long-term care home
