Primary health care clinic for registered patients only (not a walk-in clinic) providing chronic disease management - diabetes, hypertension, hypercholesterol, obesity * mental health counseling, assessment and referral * addictions assessment, referral, counseling, after-care counseling Team includes:- nurse practitioners
- consulting physician
- registered practical nurses
- chronic disease coordinator
- social workers
