Service Description: Community program addresses falls prevention education, program planning and policy development * program is a partnership of Middlesex-London organizations including Public Health, South West Local Health Integration Network, London Health Sciences Centre, St Joseph's Health Care London, Middlesex-London EMS, Victorian Order of Nurses - Middlesex-Elgin, and other community and health care providers

  • promotes community collaboration on fall prevention activities and distributes health promotion resources on falls prevention
  • promotes the use of evidence-based fall risk assessment tools and falls prevention interventions
  • promotes the use of referral pathways to provide consistency and smooth transitions through different services across the continuum of care
