Position: Dietitian - Permanent Part Time
Region: Elgin
Description: The Dietitian provides nutritional assessments, develops client-focused nutrition care plans, and delivers health education, counselling, and other clinical activities in accordance with the standards of practice of the College of Dietitians of Ontario. Working as part of an interdisciplinary team, this role supports both individual client care and broader community health initiatives, with a focus on prevention, chronic disease management, food security, and health equity. The Dietitian also contributes to program development, implementation, monitoring, and evaluation, and may participate in CCHC’s Mobile Clinic and outreach-based service delivery.
Key Responsibilities
- Conduct nutritional assessments and develop client-focused nutrition care plans
- Provide nutrition counselling, education, and clinical interventions in keeping with professional standards
- Support clients in chronic disease prevention and management through evidence-informed nutrition care
- Assess community nutrition needs and identify barriers to food security
- Facilitate and support community initiatives that improve access to food and nutrition supports
- Contribute to the development, implementation, monitoring, and evaluation of health promotion and nutrition related programs
- Participate in program development and continuous improvement activities
- Maintain timely and accurate chart documentation and participate in case conferences and chart reviews as appropriate
- Collaborate with interdisciplinary team members and make referrals as needed
- Participate in team meetings, staff development, and organizational initiatives
- Support outreach and mobile services, including participation in the Mobile Clinic, as required
- Perform other related duties as assigned
Qualifications
- Undergraduate degree in Food and Nutrition from a recognized university, or equivalent
- Completion of a Dietitians of Canada accredited Dietetic Internship, equivalent approved experience, or graduate degree in community nutrition
- Current registration in good standing with the College of Dietitians of Ontario
- Minimum three years of experience as a Dietitian in a community health or hospital setting
- Experience in program development, implementation, monitoring, and evaluation is an asset
- Demonstrated commitment to health promotion and addressing the social determinants of health • Strong assessment, communication, interpersonal, and organizational skills
- Ability to work effectively in a fast-paced environment
- Ability to respond positively to complex or demanding issues while maintaining a welcoming and professional approach
- Proficiency with computers and software applications, including electronic documentation systems and Microsoft Office • Valid Ontario driver’s licence and ability to travel locally is considered an asset
Employment Conditions
- Wage: $32.00–$37.35 per hour, based on experience and qualifications
- Permanent part-time position, 22.5 hours per week
- Eligible for HOOPP pension and prorated benefits, in accordance with CCHC policy
- Conditional upon satisfactory references and required screening
- Applicants must be legally entitled to work in Canada
- The normal work week consists of 22.5 hours, with flexibility to include occasional evening and/or weekend hours
- Satisfactory Police Criminal Record Check or Vulnerable Sector Check
- Proof of immunization for COVID-19 and measles is required, subject to applicable accommodation under the Ontario Human Rights Code
