Position:  Project Coordinator

Region:  London and Middlesex

Description:  London Family Health Team (LFHT) is seeking a highly organized and collaborative Project Coordinator to support the implementation of initiatives aligned with Ontario's Primary Care Interprofessional Team (IPCT) priorities.

Location: Hybrid (Onsite/various LFHT locations/remote)

This role will support the London Family Health Team to translate provincial and local primary care priorities into practical, measurable improvements in patient attachment, access, interprofessional care, quality improvement, and operational readiness, and improving patient and provider experience.

Responsibilities:

Project Implementation and  Coordination —
 

  • Support the planning, implementation, monitoring, and evaluation of priority primary care projects.
  • Develop project workplans, timelines, action trackers, and project documentation.
  • Coordinate meetings, activities, deliverables, and follow-up with internal and external partners.
  • Identify issues, risks, barriers, and opportunities and escalate them appropriately.
  • Support the development and implementation of standardized workflows and processes.
  • Develop a sustainment/close out and lessons learned at project’s end.

Patient Attachment, Access and Interprofessional Team-Based Care —

  • Support implementation of processes to improve access and attachment of unattached patients.
  • Support implementation of patient recruitment strategies and promoted Health Care Connect.
  • Assist with developing and refining patient intake, onboarding, referral, and triage workflows.
  • Identify opportunities to improve patient flow and the patient experience.
  • Support projects designed to optimize the use of LFHT's interprofessional team.
  • Assist with developing care pathways, referral processes, and standardized approaches to team-based care.
  • Work collaboratively with clinical and administrative staff to identify and implement workflow improvements.

Quality Improvement and Data —

  • Assist with developing sustainable processes for project measures, dashboards, reports, and presentations.
  • Track project outcomes and prepare updates for leadership and relevant committees.

Partnership and Engagement —

  • Support collaboration with the Middlesex London Primary Care Network (MLPCN) and the Middlesex London Ontario Health Team (MLOHT), primary care providers, community partners, and other stakeholders.
  • Help ensure local initiatives remain aligned with LFHT and broader primary care priorities.

Qualifications:

  • Post-secondary education in healthcare, business/administration, project management, quality improvement, or a related field.
  • Experience supporting healthcare projects, program implementation, quality improvement, or organizational change.
  • Experience working in a primary care or other healthcare environment is strongly preferred.
  • Strong organizational, project coordination, and time-management analytical and problem-solving skills.
  • Strong written and verbal communication skills.
  • Demonstrated ability to work collaboratively with interprofessional teams and diverse stakeholders.
  • Experience with data collection, reporting, spreadsheets, dashboards, or EMR data; willingness to learn healthcare information systems.
  • Proficiency with Microsoft Office, particularly Excel, Word, PowerPoint, and Teams.
  • Understanding of privacy, confidentiality, and healthcare information management requirements is an asset.
  • The successful candidate will be a practical, organized and relationship-focused project professional who can move initiatives from planning to implementation.
  • You don't need to have every answer—you need to be curious, collaborative, resourceful, and able to get things done.


French Proficiency:  French an asset

Salary:  $30.00 - $35.00 per hour

Terms of Employment:  Contract
