Listowel-Wingham and Area Family Health Team
Located in northern Perth and Huron Counties in Southwestern Ontario, the Listowel-Wingham and Area Family Health Team is a progressive team of health professionals with an integrated approach to healthcare. We are currently seeking a Nurse Practitioner, part-time (22.5 hours per week), for an 18-month contract.
Position: Nurse Practitioner
Region: Huron and Perth
Description: The NP will work in close collaboration with physicians and allied health care professionals within the Family Health Team (FHT), as well as community partners. The Nurse Practitioner will work in both the Listowel and Wingham locations. As vital members of the patient-focused team, nurse practitioners will provide comprehensive primary care services. Nurse practitioners provide acute/episodic care, health promotion, disease prevention and chronic disease management in a primary care setting with a goal to assist patients and their families to function optimally within their capacity. Nurse practitioners also assist in the ongoing development of primary care programs and resources in collaboration with other members of the FHT and other partners. The scope of practice of the FHT nurse practitioners follows the practice guidelines of the College of Nurses of Ontario.
Salary: $64.50 per hour
