  • Family Health Teams
  • Family Health Teams are a new approach to primary health care in Ontario, bringing together health care providers to work together in coordinating a patient’s care. Doctors, nurses, nurse practitioners and other health care professionals may be members of the team.
  • Telehealth Ontario
  • Telehealth Ontario is a free, confidential telephone service you can call to get health advice or general health information from a Registered Nurse.
