Service Description:

Adult day program * social, recreational, and therapeutic activities for adults 18 and over, in a group setting for part of the day * helps to support family caregivers who provide day-to-day care

meals
individualized social and recreational plans for each client

blood pressure, weight and medical monitoring (by a registered nurse or registered practical nurse)

Health Care Assistance
Medication Assistance	Medication must be in original dispensing container indicating name, medication, dosage and special instructions * up-to-date list of medication taken at home and at Day Program is required and is responsibility of client/caregiver
Mobility Assistance	Staff are trained in transfers, escorting and falls prevention * clients with canes, walker, wheelchairs and other mobility issues accommodated
Incontinence	Supports clients with bladder incontinence *clients encouraged to wear briefs or pads to support bladder control and invited to bring a spare change of clothing to be stored at the program
Foot Care	Available for a fee

Transportation	Volunteer drivers or staff can pick up clients in the morning and bring them home in the afternoon, for an additional fee
