Diabetes Coordinated Access in the South West

The following diabetes services in the South West have been made accessible through a single referral form:

• Diabetes Education (with a Certified Diabetes Education Nurse and/or Dietitian)

• Diabetic Foot Care (provided by a Foot Care Nurse)

• Specialist services for diabetes care (Endocrinology and Ophthalmology)

The Diabetes Coordinated Access project (funded by the LHIN) aims to work with diabetes education programs to develop standard work around intake, referral and placement of patients. This work has completed its pilot phase at the London InterCommunity Health Centre and the St. Joseph's Primary Care Diabetes Support Program, and now work will be focused on spreading these standards across the South West LHIN.
