Service Description:
|
Living alternatives to hospitalization in a home-like setting with staff support
- individual and/or family units
- shared living with low to moderate support
- shared living with high support
- youth (16-24) transitional shared living and individual units
- women only – individual units
- addictions supportive individual units
- justice units
Locations:
- London
- St Thomas
- Woodstock
- Exeter
- Strathroy
- Tillsonburg
Some individual units in other rural communities within Elgin, Middlesex, and Oxford counties, such as: Aylmer, Ingersoll, and West Lorne

Eligibility / Target Population:
|
Adults and youth 16 years and older with persistent mental illness who are characterized as complex with a history of lengthy and/or multiple hospital stays, have rehabilitation potential and readiness, as well as a desire for improved mental health
